SLCPD Investigating Deadly Crash in Fairpark NeighborhoodSALT LAKE CITY — The Salt Lake City Police Department is investigating a crash that killed a 41-year-old man in the Fairpark neighborhood. This investigation started at 8:56 p.m. on Sunday, April 21, 2024, when SLC911 received calls about a crash involving a car and an electric scooter at 800 West North Temple Street. The man killed is identified as Ismael Ontivaros. Based on the preliminary information, a car going west on North Temple Street hit Ontivaros as he rode the rented scooter south on 800 West. The driver, a 22-year-old man, remained on scene. The specific cause of the crash remains under investigation. Impairment may be a factor and additional investigative work is needed. Gold Cross and the Salt Lake City Fire Department provided life-saving aid to Ontivaros. He died at the hospital. No arrests or citations have been issued or made. This crash marks the fourth traffic-related death in Salt Lake City for 2024. The Collision Analysis & Reconstruction (CAR) Team and SLCPD Crime Lab technicians responded to the scene. The CAR Team is a multi-jurisdictional crash team that investigates crashes involving serious physical injuries or death. Members of the CAR Team receive specialized training in collision examination, speed analysis, vehicle dynamics, occupant kinematics, scene photography, surveying equipment, crash scene mapping, evidence collection and computer aided drawing programs. The CAR Team includes law enforcement from the Salt Lake City Police Department and several other Salt Lake Valley law enforcement agencies.
