SLCPD Releases Body-Worn Camera Footage Following OICI Protocol Activation

SALT LAKE CITY — Today, in compliance with Salt Lake City ordinance 2.10.200, the Salt Lake City Police Department (SLCPD) is releasing body-worn camera (BWC) footage from a case that resulted in an officer involved critical incident (OICI) protocol activation on October 29, 2025.

Based on the preliminary investigation, SLC911 received a call from a man who said their friend had been attacked in the middle of the street near 2100 South and 300 West.

As the call taker was trying to confirm their location the man said the person who was attacked was “starting to wake up”. As the call taker continues to try and get information the call disconnects.

The call taker calls the number back, the same man answers, says send somebody then says bye and hangs up again.

Two officers responded to the area and spoke with several people who confirmed some sort of altercation occurred.

Officers spoke with 47-year-old Jose Hernandez who was sitting on the curb. Officers offered Hernandez medical attention, which he declined.

Officers tried to find out if Hernandez had been assaulted or been involved in a fight, but he would not answer their questions.

Suddenly Hernandez put landscaping rocks into his hooded sweatshirt pouch.

An officer told Hernandez to put the rocks back, instead he stood up and began to walk away, ignoring commands to sit down.

Hernandez took one of the rocks out of his sweatshirt and held it in his right hand. He refused to drop the rock despite commands from officers.

An officer turns on his Conducted Energy Device (CED). Hernandez turns back towards the officer, then begins to walk away.

Officers told bystanders to move as Hernandez started to walk south along the sidewalk.

Hernandez ran across 300 West. As Hernandez got to the corner of 2100 South 300 West he turned and threw a rock at an officer. The officer fired at least one round hitting Hernandez.

Officers detained Hernandez, began providing lifesaving aid and asked for medical to expedite.

Salt Lake City Fire and Gold Cross responded and took Hernandez to a hospital where he died.

The officer sustained a non-life-threatening injury and was treated at a local hospital.
